REFERRAL
A referral and a mental health plan are not necessary. However, after communication with myself it may be evident that this service is not right for your needs.In this event, I can offer you some alternative options.

SESSION TIMES
Standard appointment times are 50-55 minutes in length leaving 5 minutes to settle your payment for service and make bookings for future sessions. For couple and family therapy the first appointments can be up to 90 minutes in length.

CONFIDENTIALITY & PRIVACY CONDITIONS

Purpose of collecting and holding information

The information gathered will be relevant to counselling and treatment. The information is retained to document the counselling process and inform treatment.

Access to client information

At any stage, you can access your file information through the Freedom of Information ACT. Under law client your file will be held for 3 years.

Confidentiality

Your personal information will remain confidential unless:

  1. It is subpoenaed by the courts, or
  2. Disclosure of information that would place you or another person at serious or imminent risk.

SESSION FEE
Payment for service is to be paid following your session.
Please note that I do not offer a Medicare rebate.
A Mental Health Plan is not required to access my services. I do not offer a medicare rebate.
NB: Family and Couple Therapy are not covered under the Medicare rebate scheme. I am a registered Bupa and Medibank provider and you may be able to claim for your service. Should you wish to access financial assistance through your insurer you will require my provider no. please email and I will provide you with this information.
Late Cancellation Fee’s do apply.
 ( Upon making a booking with Danielle she will email you her terms of business for your review and completion. This paperwork must be thoroughly read, completed and returned to Danielle before your service).

LATE CANCELLATION FEE
There are usually people on a waitlist wanting to access counselling. If you give enough notice when cancelling or changing appointments this allows time for those people to access services sooner. You may yourself be one of those people hoping to use a cancelled appointment opening, so please bear this in mind when changing and cancelling appointments.

If, for some reason, you need to cancel or postpone the appointment, please give at least 24 hours’ notice. If you do not provide 24 hours’ notice to cancel or rearrange your appointment, this will result in you being charged the full service fee.

PAYMENT OPTIONS
Payment for service will be required following your session.

Due to Covid-19 Payment for service will be required to be made now via direct
• via direct deposit ( you will be invoiced for your session, if you do not pay you will incur an additional fee).
